Cooling heads and hearts versus cooling our heels.

چکیده
Evidence suggests that full implementation of therapeutic hypothermia after cardiac arrest could save thousands of lives each year. Critical unanswered questions persist, beginning with a lack of evidence in regard to how early cooling should be initiated, which cooling strategy is most effective at cooling the brain, and, ultimately, what method of cooling is most optimal for survival. Two reports in this issue of Circulation, from Castren et al1 and Bernard et al,2 begin to provide some of these needed data. Given the immense logistic difficulty of performing randomized field studies, the teams of investigators involved in both studies are to be congratulated for conducting this important research. The presented results begin to explore the important question of how quickly and by what methods cooling should be started in the field. Although they provide some answers, they also provoke new questions. For clinicians using therapeutic hypothermia with cardiac arrest patients, it is important to clearly understand the pros and cons of earlier cooling initiated in the field with the newer techniques explored in these 2 reports. It is also important to understand the differences between the findings in these studies, along with the practical implications and limitations of each technique, before conclusions are drawn about the manner in which these studies may influence practice.

Myocardial protection in chronic volume-overload hypertrophy of immature rat hearts.
OBJECTIVE The benefit of cardioplegic cardiac arrest for protection of the immature myocardium is controversial. We therefore investigated the efficacy of (1) topical hypothermia alone (2) slow cooling by coronary perfusion hypothermia and (3) cardioplegic cardiac arrest plus topical cooling for protection of isolated immature rat hearts (age: 28 days). METHODS The isolated perfused rat heart...
متن کاملHow to Design a Cryogenic Joule-Thomson Cooling System: Case Study of Small Hydrogen Liquefier
Heat exchangers are the critical components of refrigeration and liquefaction processes. Selection of appropriate operational conditions for cryogenic recuperative heat exchanger and expansion valve operating in Joule-Thomson cooling system results in improving the performance and efficiency. In the current study, a straightforward procedure is introduced to design an efficient Joule-Thomson co...
متن کاملA Risk for Nosocomial Infections: Contamination of Hospital Air Cooling Systems by Acantamoeba spp.
Background and Objectives: In the most Iranian hospitals the internal air is cooled by a system comprising centrifugal blower fan and excelsior filters. Because amoeba can be present in air and dust it may enter into filters of cooling systems. It has been suggested that diffusion of amoebae cyst through air canals may contribute to various types of nosocomial infections. This ...
متن کاملA study of the myocardial protective effect of rapid cooling based on intracellular Ca, intracellular pH, and HSP70.
OBJECTIVE It has been reported that rapid cooling of the heart during normothermic coronary circulation at reperfusion after ischemia promotes early recovery of cardiac function due to the positive inotropic effects on the myocardium produced by cooling. The aim of the present study was to investigate the myocardial protective effect of rapid cooling by measuring heat shock protein (HSP) levels...

Abstract Background: Heat exposure is one of the harmful agents at work environments. Workers in high temperature environments are susceptible to heat strain. The purpose of this study was to evaluate the performance of Iranian cooling vests containing phase change material in hot and humid conditions in the Persian Gulf. Materials and Methods: This interventional study carried out on 6 subj...
